HOLLY PATTERSON SKILLED NURSING FACILITY, AND REGIONAL COMMUNITY HEALTH CENTERS BY RAISING AWARENESS OF THE ROLES OF THESE PUBLIC INSTITUTIONS AS THE REGION'S HEALTH CARE SAFETY NET FOR THE MOST VULNERABLE POPULATIONS.
