BCP IS A NEW MODEL OF INNOVATIVE PEDIATRIC HEALTHCARE THAT ADDRESSES THE SOCIAL DETERMINANTS OF HEALTH WITHIN A RELATIONSHIP-DRIVEN FRAMEWORK.
