TO PROVIDE MENTAL HEALTH COUNSELING, TREATMENT AND THERAPY AND PROVIDE LONG-TERM RESIDENTIAL CARE TO PERSONS WITH CHRONIC ILLNESS OR DISABILITY IN A HEALTH CARE CENTER (FORMERLY REFERRED TO AS A NURSING HOME).
