AND AFFILIATED ORGANIZATIONS DUE TO THE MERGER WITH BAPTIST HEALTH SOUTH FLORIDA. THE BOARD OF DIRECTORS ADOPTED A RESOLUTION TO DISSOLVE THE ORGANIZATION ON JANUARY 28, 2021 AND THE ARTICLES OF DISSOLUTION WERE FILED WITH THE FLORIDA SECRETARY OF STATE ON APRIL 7, 2021.
