TO PROVIDE DIRECT PLACEMENT OF HOMELESS AND FORMALLY HOMELESS FAMILIES AND INDIVIDUALS INTO TEMPORARY SHELTER AND TRANSITIONAL HOUSING UNITS, THAT ARE SUPPORTIVE AND PROVIDE ONSITE CASE MANAGEMENT SERVICES, FAMILY AND INDIVIDUAL COUNSELING, AND REFERRAL SERVICES. THE ORGANIZATION IS ALSO MISSIONED TO CONDUCT HOMELESS OUTREACH SERVICES, HEALTH AND WELLNESS SERVICES, ON-SITE CASE MANAGEMENT SERVICES.
