TO LEAD THE TRANSFORMATION OF PRIMARY CARE IN RHODE ISLAND IN THE CONTEXT OF AN INTEGRATED HEALTH CARE SYSTEM.

Part 3 - Line 4a
CARE TRANSFORMATION COLLABORATIVE OF RI CONTINUED TO OPERATE REMOTELY DUE TO THE PANDEMIC. WE RECEIVED CARES ACT FUNDING TO SUPPORT A NUMBER OF KEY PRIORITIES RELATED TO THE DELIVERY OF HIGH QUALITY PRIMARY CARE. PEDIATRIC RELIEF FUNDING WAS PROVIDED TO SUPPORT PRIMARY CARE PRACTICES IN ACHIEVING PRE-COVID IMMUNIZATION RATES FOR CHILDREN GOING BACK TO SCHOOL. THE DIABETES HEALTH EQUITY CHALLENGE WAS LAUNCHED TO FOCUS ON IMPROVING OUTCOMES FOR PATIENTS WITH DIABETES LIVING IN UNDERSERVED REGIONS OF THE STATE. THE COMMUNITY HEALTH TEAMS CONTINUED TO SERVE HIGH RISK PATIENTS IN THE COMMUNITY DELIVERING FOOD AND SUPPLIES NEEDED DURING THE COVID 19 CRISIS. LEARNING COLLABORATIVE MEETINGS CONTINUED REMOTELY WITH GREAT SUCCESS DESPITE THE PUBLIC HEALTH EMERGENCY. INTEGRATING BEHAVIORAL HEALTH IN PRIMARY CARE PRACTICES CONTINUED THROUGHOUT THE YEAR AND TELEHEALTH SERVICES WERE WIDELY IMPLEMENTED.
