HEART OF OHIO FAMILY HEALTH CURRENTLY OPERATES AT FIVE LOCATIONS: HEART OF OHIO FAMILY HEALTH AT CAPITAL PARK, HEART OF OHIO FAMILY HEALTH AT JAMES B. FEIBEL CENTER, HEART OF OHIO FAMILY HEALTH AT WHITEHALL - YEARLING HIGH SCHOOL, HEART OF OHIO FAMILY HEALTH AT MOUNT CARMEL EAST HOSPITAL AND HEART OF OHIO FAMILY HEALTH AT CANAL WINCHESTER. IN 2021, THE ORGANIZATION PROVIDED SERVICES TO 14,759 INDIVIDUAL PATIENTS, AN INCREASE OF 31% PATIENTS FROM 2020. THE HEALTH CENTER PROVIDES PRIMARY MEDICAL CARE, OBSTETRIC AND GYNECOLOGICAL CARE, PSYCHIATRY AND BEHAVIORAL CARE, SUBSTANCE USED DISORDER SERVICES, NUTRITIONAL SERVICES, CLINICAL PHARMACY, LABORATORY SERVICES AND OTHER ENABLING SERVICES SUCH AS TRANSPORTATIONS, INTERPRETATION, AND CASE MANAGEMENT. SOME OF OUR 2021 ACCOMPLISHMENTS INCLUDE IMPROVEMENT IN CANCER SCREENING, COLORECTAL CANCER FROM 28% SCREENING RATE IN 2020 TO 40% SCREENING RATE IN 2021. BREAST CANCER SCREENING FROM 27% TO 40% IN 2021 AND CERVICAL CANCER SCREENING FROM 63% TO 70%. THE IMPROVEMENT EFFORTS STEM FROM COLLABORATIVE WORK WITH THE AMERICAN CANCER SOCIETY TO REDUCE CANCER CARE DISPARITY IN THE MINORITY COMMUNITIES. WE ALSO IMPROVED OUR HIV SCREENING FROM 22% TO 40% IN 2021. WE ALSO DEMONSTRATED WITH CLINICAL OUTCOME EVIDENCE THAT HEALTH CARE DISPARITY IN DIABETES AND HYPERTENSION CAN BE IMPROVED BY PROVIDING CULTURALLY COMPETENT CARE. AT THE END OF 2021, THE CLINICAL OUTCOMES IN DIABETES WERE 19% CONTROLLED COMPARED TO 23% IN 2020 AND HYPERTENSION CONTROL WAS 74% CONTROLLED COMPARED TO 71% IN 2020. AS SUCH HRSA, AWARDED HEART OF OHIO FAMILY HEALTH WITH AN AWARD RECOGNITION OF QUALITY HEALTHCARE LEADER - THE FIRST OF SUCH RECOGNITION IN CENTRAL OHIO FQHC'S. THE AMERICAN HEART ASSOCIATION AND THE AMERICAN MEDICAL ASSOCIATION ALSO RECOGNIZED US WITH A GOLD PLUS AWARD IN HYPERTENSION CONTROL. HEART OF OHIO FAMILY HEALTH ALSO CONDUCTED A MENTAL HEALTH STIGMA REDUCTION CAMPAIGN TO THE SOMALI-SPEAKING INDIVIDUALS SERVING 250 INDIVIDUALS AND HISPANIC / LATINA COVID-19 VACCINATION CAMPAIGN TO BOOST VACCINATION IN OUR HISPANIC COMMUNITY. WE ALSO PARTICIPATED IN THE LOCAL VACCINE FOR CASH CAMPAIGN TO INCREASE RATE OF COVID-19 VACCINE IN THE BLACK COMMUNITY. BY DECEMBER 31, WE HAVE ADMINISTERED 13,127 COVID-19 VACCINE TO 5,189 INDIVIDUALS.
