SHELTER: SHELTER IS OFFERED TO EIGHTY SIX ADULTS EVERY NIGHT WHILE WE WORK WITH THEM TO ACCESS HOUSING AND SUPPORT SERVICES. IN FISCAL YEAR 2022, 408 INDIVIDUALS WERE PROVIDED SHELTER BETWEEN BOTH SHELTER LOCATIONS.


RAPID REHOUSING PROGRAMS - THESE PROGRAMS HELPS FAMILIES AND SINGLE ADULTS TRANSITION QUICKLY FROM SHELTER INTO HOUSING WITH SUPPORTIVE SERVICES AND RENTAL SUBSIDY. 44 SINGLE ADULTS, AND 258 FAMILIES INCLUDING 559 CHILDREN WERE SERVED IN FISCAL YEAR 2022.NEXT STEP ASSESSORS CONDUCTS COORDINATED ENTRY SYSTEM HOUSING ASSESSMENTS FOR FAMILIES IN HENNEPIN COUNTY. IN FISCAL YEAR 2022, 683 ASSESSMENTS WERE COMPLETED WITH FAMILIES. HOUSING AND SERVES UP TO 140 SINGLE ADULTS AND FAMILIES.


SUPPORTIVE HOUSING PROGRAMS PROVIDE LONGTERM, SCATTERED-SITE HOUSING AND LONG-TERM SUPPORTIVE SERVICES FOR FAMILIES AND SINGLE ADULTS WHO HAVE EXPERIENCED LONG- TERM HOMELESSNESS AND HAVE A DOCUMENTED ELIGIBLE DISABILITY. IN FISCAL YEAR 2022, 174 FAMILIES, INCLUDING 277 ADULTS AND 401 CHILDREN, AND 295 SINGLE ADULTS RECEIVED LONG-TERM CASE MANAGEMENT.MINNESOTA HOUSING FINANCE AGENCY HOUSING TRUST FUND SUBSIDIES RENTAL SUBSIDIES ARE AN IMPORTANT TOOL FOR HELPING SOME SINGLE ADULTS AND FAMILIES MAINTAIN THEIR HOUSING WHILE THEY WORK TOWARD SELF-SUFFICIENCY. THESE SUBSIDIES ARE PROVIDED THROUGH MINNESOTA HOUSING AND SERVES UP TO 140 SINGLE ADULTS AND FAMILIESPERMANENT SUPPORTIVE HOUSING PROGRAMS - FAMILY AND SINGLE ADULT FAST TRACK IS A CONDENSED FORM OF RAPID REHOUSING FOR FAMILIES AND SINGLE ADULTS WITH HIGHER INCOME. THIS PROGRAM OFFERS ASSISTANCE WITH THE HOUSING SEARCH, APPLICATION FEES, AND REFERRALS TO BRIDGING FOR FURNITURE, BUT NO ONGOING RENTAL ASSISTANCE OR CASE MANAGEMENT. 66 FAMILIES, INCLUDING 95 ADULTS AND 151 CHILDREN, AND 45 SINGLE ADULTS WERE SERVED IN FISCAL YEAR 2022.
