ELIMINATE BLIGHTED HOUSING CONDITIONS IN THE CITY OF ALLENTOWN'S NEIGHBORHOODS AND TO INCREASE AVAILABILITY OF HOUSING OPPORTUNITIES FOR LOW INCOME RESIDENTS BY PURCHASING VACANT, DETERIORATED RESIDENTIAL PROPERTIES AND TOTALLY REHABILITATE THE UNITS FOR RESALE OR RENT.

These are the most common type of private foundation. They are generally endowed, usually from a single individual or family. THE NAME CHANGE IS THE RESULT OF LAUNCH'S EXIT FROM THE NATIONAL NETWORK OF COMMUNITIES IN SCHOOLS, INC., WHICH BECAME EFFECTIVE DECEMBER 31, 2019. MISSION: LAUNCH'S MISSION IS TO SURROUND STUDENTS WITH A COMMUNITY OF SUPPORT, EMPOWERING THEM TO STAY IN SCHOOL AND ACHIEVE IN LIFE. LAUNCH FULFILLS ITS MISSION BY ENTERING INTO SERVICE CONTRACTS WITH THE SCHOOL DISTRICT OF PHILADELPHIA AND COMMUNITY ORGANIZATIONS.

OUR PROVEN, SUSTAINABLE, AND SCALABLE MODEL THAT BRINGS TOGETHER FAITH AND SCIENCE IS BASED ON THE ESTABLISHMENT OF A TRAINED FAITH-BASED HEALTH NAVIGATOR IN EACH CHL CHURCH.WE'RE AT THE FOREFRONT OF SHAPING THE HEALTH EQUITY FIELD TO ADDRESS SOCIAL DETERMINANTS OF HEALTH, AS WELL AS A LEADER ON THE GROUND IN PROVIDING HEALTHCARE SERVICES AND HANDS-ON ACTION WITH VULNERABLE COMMUNITIES. WE AREN'T JUST DISCUSSING THE PATH TOWARDS HEALTH EQUITY - WE'RE PAVING IT.FOUNDED IN 2021 DURING THE HEIGHT OF THE COVID PANDEMIC, CHOOSE HEALTHY LIFE FUNDED 120 CHURCHES ACROSS 13 STATES THAT CONDUCTED OVER 3,700 EVENTS AND VACCINATED AND TESTED OVER 110,000 INDIVID
